Nurse practitioner certification, exam by exam

A new nurse practitioner sits one national board exam in the population the program trained them for, and the state board of nursing licenses them in that same population. Fifteen such exams from seven certifying bodies are tracked here, grouped by the patients you will see. First, a renaming: the board known for years as AANPCB now operates under the brand NPCB, so "the AANP exam" means NPCB's FNP-C.

Exams with a code link have their own section on this site: outline, study plan and a practice set with every option explained. The quieter rows are facts only for now. One retired credential still turns up in searches: ANP-BC is renewal only, with 14,058 nurse practitioners still holding it at the end of 2025.

AANP, AANPCB, NPCB: one board

The American Academy of Nurse Practitioners Certification Board describes itself in its 2025 Annual Report as operating under the brand NPCB (annual report, p. 1, read Nov 7, 2026). The credentials kept their names: FNP-C, AGNP-C, PMHNP-C and ENP-C. AANP, the American Association of Nurse Practitioners, is a membership group; joining it lowers the NPCB fee, but it does not award the credential. The board's exams, verification and renewal are on the NPCB page, and the choice between NPCB and ANCC is on AANP vs ANCC.

A 70-year-old man reports unsteadiness that is worse in the dark. During your neurologic examination, which test best assesses proprioceptive (dorsal column) function contributing to balance?

  1. Romberg removes vision: a patient who stands steadily with eyes open but sways or falls once they close has lost proprioceptive (dorsal column) input, which fits imbalance that worsens in the dark.
  2. Reflexes test the segmental reflex arc; they can be reduced in neuropathy but do not measure the position sense that balance in the dark relies on.
  3. Finger-to-nose detects cerebellar dysmetria and intention tremor, a different cause of unsteadiness that does not depend on whether the eyes are open.
  4. Rapid alternating movements test cerebellar coordination (dysdiadochokinesia); they stay abnormal with eyes open and do not isolate joint-position sense.

A 2-year-old has had weight faltering since 12 months with abdominal distension, frequent loose foul-smelling stools, and irritability. The child eats wheat products daily. Which test is MOST appropriate to screen for the suspected diagnosis?

  1. Chronic diarrhea, distension, and weight faltering in a toddler eating gluten point to celiac disease. The screen is tTG-IgA while gluten is in the diet, paired with a total IgA level so that IgA deficiency cannot hide a true positive.
  2. HLA typing is useful to rule celiac disease out, because almost everyone with it carries DQ2 or DQ8, but a positive result is common in the general population and cannot screen or confirm.
  3. Ordering tTG-IgA without a total IgA misses the child with IgA deficiency, which is more common in celiac disease and makes the tTG-IgA falsely negative.
  4. Sweat chloride is the test for cystic fibrosis, the malabsorption differential that usually adds recurrent respiratory infections and often shows up in infancy or on the newborn screen.

A ventilated 3-week-old with evolving BPD receives PN at a GIR of 16 mg/kg/min with lipid 2 g/kg/day. PaCO2 has risen from 50 to 62 mmHg with no change in lung findings. The most likely nutritional cause is

  1. Blaming the lipid is a common reflex, but 2 g/kg/day does not raise PaCO2; lipid has the lowest respiratory quotient of the macronutrients.
  2. Glucose given beyond the oxidative capacity of about 12 mg/kg/min is turned into fat, a process with a respiratory quotient above 1, so CO2 production rises in an infant with little ventilatory reserve; lowering the GIR helps.
  3. Protein oxidation yields less CO2 per calorie than glucose, so it cannot explain a CO2 rise that tracks a very high GIR.

Which medication is FDA-approved and considered first-line for bulimia nervosa?

  1. Fluoxetine is the only medication FDA-approved for bulimia nervosa, and its labeled dose of 60 mg/day is higher than the usual depression dose.
  2. Lisdexamfetamine is FDA-approved for moderate to severe binge-eating disorder, not bulimia; this choice mixes up the two binge-eating conditions.
  3. A benzodiazepine adds dependence risk and does nothing for the binge-purge cycle; no benzodiazepine is indicated for bulimia.
  4. Bupropion is contraindicated in current or past bulimia or anorexia because purging-related electrolyte shifts raise its seizure risk; choosing it for weight effects is a dangerous error.

WHNP-BC · Gynecologic disorders

A 58-year-old has dyspareunia from genitourinary syndrome of menopause. Lubricants have not helped, she declines any estrogen product, and she prefers an oral medication. The most appropriate option is

  1. Ospemifene is an oral selective estrogen receptor modulator that acts as an estrogen agonist on vaginal epithelium and is FDA-approved for moderate to severe dyspareunia from menopause, so it fits a patient who wants a pill but no estrogen product.
  2. Flibanserin treats acquired hypoactive sexual desire disorder in premenopausal women; choosing it confuses low desire with pain from atrophic tissue, which it does not reverse.
  3. Oral estradiol would relieve atrophy, but it is an estrogen product she has declined; systemic estrogen is also more than isolated genitourinary symptoms need.

AOCNP · Cancer Continuum · 28%

A 72-year-old's colon adenocarcinoma shows loss of MLH1 and PMS2 on immunohistochemistry, and a BRAF V600E mutation is detected. The most likely cause of the mismatch repair loss is:

  1. BRAF V600E in a tumor with paired MLH1/PMS2 loss points to acquired promoter methylation that silences MLH1, the usual mechanism in sporadic MSI-high colon cancer of older adults, so germline testing is usually not needed unless the family history is strong.
  2. PMS2 disappears in this pattern only because it needs MLH1 to stay stable; a primary PMS2 defect shows isolated PMS2 loss with MLH1 intact.
  3. This is the Lynch syndrome explanation for MLH1/PMS2 loss, but it overlooks the BRAF V600E result, which is rarely found in Lynch-associated colorectal cancers and points to a sporadic cause.
  4. Paired loss of MLH1 and its partner PMS2 is a coherent biological pattern, and the BRAF result supports a real defect; dismissing it as artifact discards an MSI-high tumor.

The boards count differently. NPCB, ANCC, PNCB and AACN report first-time takers; NCC divides by everyone tested; ONCC reports initial candidates. Two figures are older than the rest: PNCB had not posted its 2025 report on the pages we could read, so CPNP-PC and CPNP-AC show 2024, and DNPCB posts no rate at all. The FNP-C series by year is on the FNP-C passing score page, and the ANCC psychiatric series on PMHNP-BC pass rate.

Upkeep varies even more from board to board than the exams themselves:

Preparing for the board you picked

Start from the outline your board uses now, not from a review book's chapter list. Two family outlines changed recently:

  • FNP-C (NPCB): 2024 blueprint, the version in use
  • FNP-BC (ANCC): Oct 30, 2026, the date the new outline took over

Each study guide here splits weeks by the outline's own shares:

Where two exams overlap, the shared topic is written up a single time, under the population it belongs to: childhood asthma on pediatric respiratory, contraception on women's health contraception, adult screening on FNP-C preventive screening.

Checking that an NP is board certified

The certifying body is the primary source, and each runs its own lookup:

  • NPCB: online verification; free when sent to a state board of nursing, paid when an employer asks (FNP Renewal Handbook, June 4, 2026, PDF, p. 14). More on the NPCB page.
  • ANCC: ordered through an online account; no charge for boards of nursing, institutions pay per request (ANCC verification page, read Nov 7, 2026). More on ANCC NP certification.
  • PNCB, NCC, ONCC, AACN, DNPCB: the verification page on each board's own site. A wallet card or wall certificate is not proof of current status.

Questions about NP board certification

How often does an NP board certification need renewing?

It depends on the board: every 5 years at NPCB, ANCC and AACN, every 4 years for AOCNP, every 3 years at NCC and DNPCB, and yearly inside a 7-year cycle at PNCB. The table under How long a credential lasts lists each exam.

Do I need board certification to work as a nurse practitioner?

The APRN Consensus Model asks that education, certification and licensure match in role and population focus (NCSBN, Consensus Model for APRN Regulation, July 7, 2008, PDF p. 10). Your state board of nursing decides which certifications it accepts, so check its list before you apply.

Can I sit an NP board exam at home with a remote proctor?

For three of the fifteen: the two NCC exams (test center or remote) and AACN's ACNPC-AG. NPCB and ANCC test at Prometric centers, and PNCB, ONCC and DNPCB through PSI.

Which NP exam is the longest?

DCNP: 4 h for 175 questions. The ANCC and AACN exams run 3.5 h for 175; NPCB's run 3 h for 150.
